DOT Physical Exams at Melusi Centers
At Melusi Centers, we provide comprehensive Department of Transportation (DOT) physical exams designed to help you meet federal requirements and stay on the road.
Whether you’re renewing your certification or completing your first exam, we aim to make the process efficient, respectful, and straightforward.

A DOT physical is a federally required medical exam for commercial drivers, regulated by the Federal Motor Carrier Safety Administration (FMCSA).
The purpose of the exam is to assess whether a driver is medically qualified to safely operate a commercial motor vehicle.
During your visit, we will evaluate:
Vision and hearing
Blood pressure and cardiovascular health
Neurologic function
General physical health and mobility
Medical history and current medications
Based on the findings of your exam, a Medical Examiner’s Certificate may be issued in accordance with FMCSA guidelines.

You may need a DOT physical if you:
Operate a commercial motor vehicle (CMV)
Transport more than 15 passengers
Transport hazardous materials
Drive a vehicle with a gross vehicle weight rating (GVWR) of 10,001 pounds or more
Are required by your employer or state regulations to maintain certification

DOT Physical Exam: $150
This includes your exam and completion of all required documentation.
While the primary purpose of the DOT exam is certification, your visit can also provide helpful insight into your overall health.
If any potential concerns are identified, we will recommend follow-up with your primary care provider (PCP). We also offer optional additional testing and lab review consultations (for an additional fee) for those who would like a more in-depth evaluation.
