Health Coaching
Health coaches help individuals manage chronic illness in a variety of ways such as improving diet and nutrition, exercise, sleep, and stress management. They also provide education for patients and serve as a connection between the patient and the provider.
Health Coaching is a collaborative process which provides support to individuals who desire to make changes in their lives. Health coaches use evidence-based practices to identify goals, create a plan of action, check in on progress and identify barriers and offer solutions. Motivation and self-empowerment are two key principles health coaches use to help individuals facilitate change and adopt healthy habits.
